Interview Questions to weed out candidates

What are some sample interview questions that you use to weed out prospective mentors to determine if they have temperment, personal characteristics and commitment to serve as a mentor in your program?

And, I understand that many mentoring programs conduct the in-person interview with two staff people to have two different perspectives on the candidate? Do smaller programs so this or is this practice mainly used by larger agencies?
