Tool for evaluating outcomes for Young Adults involved in Mentoring Programs???

Hi, I coordinate a mentoring program in partnership with the Metropolitan Fire Brigade (MFB) in Melbourne, Australia. The program aims to train 20 operational fire fighters to mentor young people from the age of 16 to 30 from Aboriginal or Culturally and Linguistically Diverse backgrounds as the MFB is under represented by these groups. The program will be 1:1 community based mentoring with a vocational element. I was wondering if anyone knows of a tool for evaluating outcomes for young adults participating in a mentoring program. There seems to be alot of surveys out there for youth but it would be helpful to see what measures other programs working with young adults have in place.
If anyone can help me it would be greatly appreciated.
